Membership Transfer Request Form
Please use this form only if you are requesting membership transfer INTO the Palm Bay Seventh-day Adventist Church. If you are requesting a membership transfer OUT OF the Palm Bay SDA Church, please have the church clerk of your new church request the membership transfer via eAdventist. If your church is located outside of the North American Division, please have the clerk of your new church email the membership transfer request to headclerk@palmbaysdachurch.com. The online membership transfer systems unfortunately do not work across divisions, so an emailed request is needed.
